PRINCIPLE OF THE TEST.
Accessioning into a permanent logbook is performed daily by the
Histology Technician, for organized record-keeping of all specimens
that come through the Histology Laboratory.
SPECIMEN REQUIRED.
All human tissue excised at surgery, outpatient clinics,
fresh or in fixative, along with a filled-out Tissue Examination Form
(U. S. Standard Form 515, USSF515).

STEP-BY-STEP DESCRIPTION.
1. Accessioning is performed daily by the Histology Technician.
Residents may accession late arriving cases for which there is
a clinical need to process the specimen as soon as possible.
2. The paper accessioning logbook is located
in the Histology Laboratory, Room 4D-124.
The book consists of column numbers, as follows:
2.1. The date and surgical pathology accession number.
2.2. Last four digits of the patient's Social Security Number.
2.3. Patient's name.
2.4. Doctor's last name and the ward number or location
of where the specimen originated, e.g., 4B, 5A, OP
2.5. Specimens that are received, what tissue,
and the number of containers received
2.6. If more space is needed, then the columns
numbered 6, 7, 8, 9, 10, 11, and 12, can be used.
3. The surgical pathology accession number is written on the specimen
container in red, e.g., BSP 08-123. Both numbers on the Tissue Examination
Form (U. S. Standard Form 515, USSF515),
and on the container should read the same. The surgical number is written
in ink on the surgical sheet in the box on the right side of the page halfway
down the center. In case one or two of the containers are sent to Cytology,
this should be noted in the book and the same numbers should be retained.
4. The containers are then lined up in numerical order on the counter
next to the grossing area; the sheets are put in numerical order.
The counter is washed down daily with bleach. The grossing area
is washed down daily by the Resident.
5. The Resident writes up the cassette beforehand. If needed,
the technician will help to write the cassettes beforehand.
5.1. Pink Cassettes: Small Biopsies, e.g., skins, GI, Lung
Biopsy, Prostate Biopsy. These cassettes are cut first.
5.2. White Cassettes: Large sections, e.g., legs, prostate
(TURPs), colons, neck resections.
5.3. Lavender Cassettes from Cytology:
These cassettes are placed in basket by the Resident
and the Cytology number is written on the surgical
grossing sheet. (Used as a record check,
verifying that the cytology specimen has been obtained).
5.4. Beige Cassettes from Fort Howard:
grossed by Resident and placed on processor.
